Provision Center for Proton Therapy – Patient Care Coordinator


The Patient Care Coordinator accepts referrals for cancer services from a variety of sources, including but not limited to, referring clinical providers, patients, and the patient’s family. The Care Coordinator will use clinical knowledge, medical protocols and direct interaction with Provision Cancer Center’s Oncology Specialists to determine the initial consult plan for the patient. The Care Coordinator will gather medical records and assist in the scheduling of necessary consultations and additional appointments. The Care Coordinator will work closely with the Nursing team and Provision Oncologists to coordinate all referral information and subsequent scheduling of visits/therapy at the Cancer Center. Additionally, the Patient Care Coordinator will work in concert with the insurance team to verify the patient’s benefits, calculate out of pocket costs and determine eligibility for having their treatment covered. Patient Care Coordinator will assist hospitality with any needs the patient may have. The Patient Care Coordinator also serves to coordinate Provision Cancer Center Multidisciplinary Tumor Conferences as needed.


• Receives inquiries from referring clinicians and/or their representatives, ensuring all calls are received or returned within specified time frame

• Collect patient information, demographics, basic diagnosis and insurance information, and record relevant marketing feedback information

• Gather self-referred patient medical records before consult

• Communicate new consults to other clinical and administrative support teams

• Educate potential patients generally about treatments, diagnostics, and other services offered on campus such as proton therapy, chemotherapy, conventional radiation therapy, clinical trials, and others

• Promote patient utilization of patient portal

• As required, presents cases to physician group during patient case review

• Performs all necessary functions as tumor conference coordinator, including establishing agendas, communicating with attendees, communicating with provider of continuing medical education, gather case information, ordering food and supplies, record keeping and follow up

• Establishes and maintains excellent patient -focused working relationships with partner hospital/physicians/referring physicians’ office and facilities, and proactively communicates with same to address problems/improve quality of process


• Bachelor’s degree required

• 2+ years’ oncology experience, preferred

• Strong communication and problem solving skills

• Ability to work well under pressure

• Must be self-motivated

• Strong team player

• Excellent organizational skills

• Experience with Electronic Medical Records, preferred

• Knowledge of Mosaiq preferred

• Familiar with medical terminology

• Proficient with MS Office suite

Able to implement policies, procedures and computer applications related to the Patient Care Coordinator position
